Notification of Intent

This form is to submit notification of intent to move from a Sales Rate (GS/FS/IS) to the Transportation Service Rate (TS). Notification of intent is required no later than end of business day on March 7th for an in-service date of July 1st of that same year. Per the Dominion Energy Utah (and Idaho) Natural Gas Tariff, the requestor must provide the required information below:
